Minnesota law requires children enrolled in school be immunized against certain diseases or file a legal medical, or non-medical exemption. Medical exemptions must be signed by a healthcare provider and/or parent/guardian. Non-medical exemptions must be signed by a parent/guardian and notarized.
If you are in the process of “catching up” with your child’s immunizations, please notify a health services staff member. We will work with you and your child to update their record as they receive immunizations.Â
Please be sure your child’s immunizations are up to date prior to the start of the school year.Â

3 Hepatitis B
5 Dtap (diphtheria, tetanus, and pertussis)
4 Polio
2 MMR (measles, mumps, rubella)
2 Varicella (chickenpox)
Immunization Requirements to Enter 7th grade
All of the above, plus:
1 Tdap at age 11-12 (Booster shot)Â
1 Meningococcal (meningitis)
