Medication Forms

If you request your child have a medication administered at school, please complete the appropriate permission form and return it to the School Nurse or health assistant. A new form must be completed at the start of each school year even though there may be no changes in the type or dose of medication.

High School students in grades 9-12 may only carry over the counter medications such as acetaminophen or ibuprofen. The medication must be at school in the original container. Please have a discussion with your student about the appropriate dose of chosen over the counter medication and that it is not to be shared with other students. A note from a healthcare provider is required if the student takes more than what is recommended on the medication packaging. 

Prescription and over the counter medications must be dropped off and picked up by a parent or guardian.

If the medication is a controlled substance, it must be picked up when requested by the school. For non-controlled substances, the school district is an authorized entity to transport the medication for the purpose of destruction of the unused or abandoned medications.
